Details:

1. Book your pickup appointment through the form below.
2. We offer curbside pickup for kits.  You must remain in your vehicle at all times and wear a mask.
3. At your appointment time, please park at the Van Norman Street entrance doors. If attendant is not visible on the sidewalk, call 624-2627 to advise of your arrival.  Kits will be delivered to your vehicle by an attendant.
